Your Guide to Necessary Documentation to Apply for Medicare

The process and paperwork required to apply for Medicare is long and relatively complicated. It requires a lot of documentation along the way. You can obtain some documents online; others you may need to request from your previous employer.

This guide will provide an overview of the documentation you need to qualify for Medicare and any other application options you have. 

Required Documents for Medicare Application 

When you’re ready to apply for Medicare online, you’ll need the following documents:

Valid Social Security Number (SSN)

The SSN is a unique number assigned to you by the Social Security Administration (SSA). It’s used to keep track of your earnings and other income so that Medicare can calculate how much you need to pay for medicare. Without a valid SSN, it would be impossible for Medicare to determine what benefits are available to you. Call the Social Security Administration or visit a local office if your SSN is invalid or you are having trouble locating it.

Social Security taxes fund Medicare. The SSA uses your SSN to keep track of how much money you earn so they can figure out how much you will owe for Medicare Part B. If your SSN is invalid or unavailable, then Medicare can’t determine what you owe for Medicare.

Proof of U.S. Citizenship or Immigration Status

Medicare is a federal program that provides health insurance for people age 65 and older and certain disabled Americans. To qualify, you must meet specific requirements regarding your age and income level. If you don’t have proof of citizenship or immigration status when applying for Medicare, it could delay the process significantly.

If you were born in the U.S., your birth certificate would suffice. If not, you’ll likely need another form of proof of citizenship or immigration status. You may be asked for a passport, naturalization certificate, or other documents as proof.

Proof of Identity

You will need to provide proof of identity when you apply for Medicare. This is because the program requires that only U.S. citizens, legal residents, and other individuals are eligible to receive benefits. This can be any one of the following documents:

  • Driver’s license or ID card issued by a state, county, or territory of the United States (or by a Canadian province)
  • Valid U.S. passport
  • Unauthorized Immigration Form I-551 (Permanent Resident Card)

Proof of Residence

If you are applying for the first time or your card has expired, you must provide proof of residence. This can be any one of the following documents:

  • A utility bill issued within the last 90 days showing your name and current address
  • A recent bank statement, paycheck stub, or government document with your name and current address

If you are a permanent resident, provide your Form I-551 (Permanent Resident Card). If you have an unexpired foreign passport with an endorsement from the Department of Homeland Security (DHS), that will also be proof of residence.

Proof of residence is needed to prove that you are a U.S. citizen or legal resident and therefore eligible for Medicare benefits. If you cannot prove that you are a legal resident, your application will be denied, and any money paid out as part of the application will be refunded.
